How to Obtain a Credit Application and/or Credit Report

Obtaining a credit application and/or a credit report is a standard business practice when establishing a credit relationship with individuals (consumer credit report) or companies (commercial credit report) and can help your department minimize credit risk. The Student Financial Services Collection Office can help you obtain and interpret credit reports. BYU is not always legally required to get customer authorization to obtain a credit report. However, most businesses and individuals appreciate the opportunity to approve such a request in advance. Credit reports are also helpful when entering into other business transactions which do not necessarily include the extension of credit.

Commercial Entities

If you are establishing credit with the commercial entity, obtain an Application for Commercial Credit with BYU that you can send to your proposed customer. This application also includes authorization to obtain a credit report.

If you are not establishing credit with the commercial entity, you will just need the Commercial Credit Report Authorization.

To order a Consumer Credit Report, please call or email Glenda Tolman, Collection Manager at 801-422-6634 or glenda_tolman@byu.edu. Consumer Credit Reports have special rules that Glenda will explain prior to ordering the report for you.
