Federal Regulation (OMB Circular A-21) requires that all salary and wage payments made on Federal Research Grants and Contracts be supported by an after-the-fact Time and Effort report signed by the employee or other responsible official. These reports are sent to you at the completion of each semester. We have strived to give you as much information as possible.

The information shown is a result of comparison between the original approved contracts and payments actually made to the student. If our information is not correct, please make the necessary corrections. If the student was paid from more than one account, we usually do not know the type of work they performed and need you to provide this information.

The form must account for 100% of the compensated work performed by the student. Therefore, even though most student employees work part-time, the total work they perform represents 100%. Please do not include a percentage for time spent studying or going to class. The regulations require reporting only the work they are compensated for. It is essential that we receive these reports back as soon as possible. We have therefore sent them directly to you, the Principal Investigator, instead of trying to locate the students. Please complete the form with the information requested and return to Kathleen Rugg, Financial Services, A-286 ASB by February 26, 1999.

Please categorize the type of work performed by using the following definitions.

1. a. Regular Instruction - Regularly assigned classroom teaching assignments including preparation, work in the classroom, grading of papers, and all time related to teaching.
   b. Other - Other Instructional assignments not included in 1a
2. Sponsored Research - Research sponsored by an external funding source authorized by a formal document with a work statement and budget.
3. Institutional Research - Research funded internally and separately budgeted by BYU.
4. Cost Sharing - Include in this category research effort which the student has performed as a cost sharing contribution on a sponsored research project. The cost sharing effort must be from a non-federal source. Include both the research project number benefited and the source fund/org number (where the cost share expenditure occurred).
5. Other Institutional Activities - Effort not directly related to instruction or research within the University.
