Federal Regulation (OMB Circular A-21) requires that all salary and wage payments made on Federal Research Grants and Contracts to all professional and professional staff personnel be supported by an after-the-fact Time and Effort report signed by the employee or other responsible official. These reports are sent to you for each semester. Please complete the form with the information requested and return to Kathleen Rugg, Financial Services, A-286 ASB by February 26, 1999.
We have made an effort to put as much information as we can on your report. If our information is not correct, please make the necessary corrections. Since we don't know which activities you are involved in outside of formal 5- research, please fill in all your other activities (using the definitions below). Each column should total to 100% of your time for each month. This 100% represents your full salary paid that month. Any overload (supplementary compensation) should not be included in the 100% but should be noted as the number of extra days worked. For those employees who are no longer at BYU or are otherwise unavailable, a responsible official (department chairman, supervisor, or principal investigator) who knew of their responsibilities should fill out and sign this report. The following definitions apply to items contained in Section A:
- a. Regular Instruction - Regularly assigned classroom teaching assignments including preparation, work in the classroom, grading of papers, and all time related to teaching.
b. Academic Support - Instructional assignments (not included in 1a.), research development and other scholarly activity not separately budgeted and accounted for. - Sponsored Research - Research sponsored by an external funding source authorized by a formal document with a work statement and budget
- Institutional Research - Research funded internally and separately budgeted by BYU
- Cost Sharing - Include in this category research effort which you have performed as a cost sharing contribution on a sponsored research project. The cost sharing effort must be from a non-federal source. Include both the research project number
benefited and the source fund/org number (where the cost share expenditure occurred).
- Departmental Administration. Effort related to the administrative and support service operations that jointly benefit the instructional activities, organized research, and other activities in the department.
- Other Institutional Activities - Effort not directly related to instruction or research within the University. This would include service in organizations outside the University, such as professional associations and/or civic work, where this effort is considered a part of your contribution to the general interests of the University. Include vacation in this section as well. Do not include activities for which you receive additional compensation, such as paid consulting or lecturing for pay.
